One killed, two injured in Brookfield crash

BROOKFIELD – A Pennsylvania woman is dead and two other women injured following a two-vehicle crash here Monday afternoon.

According to the Ohio State Highway Patrol, the crash occurred about 2:40 p.m. at state Routes 7 and 82.

Jessica L. Shaffer, 26, of Brookfield, received minor injuries. She was treated and released at the scene. Shaffer was driving a 2008 Hummer H3, which was heavily damaged, the patrol states.

Fadia Iraq, 32, of Sharon, Pa., received serious injuries. She was listed in critical condition at St. Elizabeth Health Center in Youngstown. Iraq was driving a 2008 Jeep Liberty, which sustained heavy damage to the passenger side.

Carmen L. Maldonado-Iraq, 73, also of Sharon, Pa., was pronounced dead at the scene. She was the right front seat passenger in Iraq’s vehicle.

At the time of the accident, Iraq was traveling northbound on Route 7 and Shaffer was traveling westbound on Route 82 and failed to stop at a red traffic light. Her vehicle struck the passenger side of the Jeep, pushing it off the northwest corner of the intersection, the patrol states.

Toxicology reports are pending; however, alcohol and/or drugs do not appear to be a factor in the crash.

Neither occupant of the Jeep was wearing a seatbelt. Shaffer was wearing her seatbelt.

Charges are pending and the crash remains under investigation by the Warren post of the highway patrol.
